Summer Support Program
Services for Youth (ages 14-21)
Community Living North Perth offers a Summer Supports program for youth aged 14-21. This program provides assistance for employment, volunteering, recreational activities and more for 8 weeks during the summer.

Passport Program
Services for Adults (ages 18+)
The Passport Program helps adults with a developmental disability to be involved in their communities and live as independently as possible. Passport Funding can be used to support individuals to take part in community classes or recreational programs; develop work, volunteer or daily life skills; hire a support worker; or get temporary respite for caregivers.
Community Living North Perth can help you manage your Passport Funding, including helping you create a plan for your funding and submitting electronic claims.

Community Participation Support
Services for Adults (ages 18+)
The Community Participation Supports Program supports adults with a developmental disability in activities such as personal development, basic life skills, social and recreational activities, volunteering and other activities that lead to community participation.
Eligibility for funded Community Participation Supports is determined through Developmental Services Ontario (DSO).

Employment Support
Services for Adults (ages 18+)
The Employment Supports program prepares and supports adults with a developmental disability in employment settings. This program prepares individuals for employment, facilitates and supports education and training, provides job training and coaching, and the supports needed to place, maintain or change employment for an individual in a work setting.

Housing Support
Services for Adults (ages 18+)
Community Living North Perth provides two types of Housing Support.
Supported Independent Living
This program supports individuals who are living by themselves or with others, but independently of family and caregivers. Individuals are supported with activities of daily living, including personal hygiene, dressing, grooming, meal preparation, medication administration and training related to money management, banking, use of public transportation, and other life skills.
Supported Group Living
This program provides 24/7 staffing support in a residence operated by Community Living North Perth. Homes accommodate groups of three or more residents, with support for activities of daily living and participation in community activities. Eligibility for Housing Support is determined through Developmental Services Ontario (DSO).

Advocacy
Services for Adults (ages 18+)
Our self-advocacy group, Self Advocates of North Perth is a group of people who have developmental disabilities and advocate for equal citizenship, acceptance and belonging in the community. This group provides a forum to discuss disabilities and the importance of inclusion in our community, with topics like “We are All the Same,” “The Power of Words and Labels,” “Removing Barriers Through Education,” etc. The Self Advocates of North Perth promote a community where all—regardless of race, religion, or abilities—share the right to be here, to live life to the fullest and to be respected and treated with dignity.
The Self Advocates meet monthly, learn how to speak up for ourselves, discover self-direction, participate in fundraising, increase public awareness, advocate the government, do presentations, engage in learning opportunities, host dances, and have fun!
